The medication staff have been observed administering medications with pudding without a doctors order. Wanda overheard making inappropriate statement to a new hire stating she didn't want to pick up another residents from the dirt in reference to a recent suicide of a resident who threw himself from the second floor. The comment was made in the presence of residents. The RP disclosed caregivers do not complete their rounds to check on resident resulting in residents sitting in there incontinence briefs for 6-8 hours.
