• Have I seen this patient myself?
• Are there standing orders I could use?
• Have I discussed the issue with my colleagues, including charge nurse (phone # 310 669) or crisis nurse (phone#310 442)?
• What I have done to resolve the problem?
• What do I expect to happen as a result of this call?
When you call use structured format (which assures consistency and accuracy):
• Situation---State what are you calling about
• Background---Identify
