An interaction between nurse and patient took place in the morning class. The client was playing with toys. The service user unable to speak but he understands what is say towards him using speech and lamh signs. There were two nurses present in the room but one of them went out and client was left with Nurse 2 who wasn’t his daily nurse. Service user needed to use bathroom. Client stopped playing ad look confused for the Nurse 1 in the room then looked at the Nurse 2. He made a growling sound to bring the attention of the nurse. Nurse 2 looked at him and she said “What’s going on”. The service user made a long eye contact with her and then he looked at the bathroom door and back onto the nurse and then he raised his hand and showed lamh sign for toilet. She ask him if he wanted to go to bathroom and he shook his head and she walked him to bathroom.

All nurses were gathered with their diaries. The office was small so the nurses sat close to each other in front of manager desk. There were 5 nurses present and a nurse manager. Manager spoke about activities planned and she mentioned what activity and what time a specific client will undertake during the week. She gave this information to the nurse that cared for this specific client. The activities that were planned were physiotherapy, communication class, dentist and bogie assessment. At the end of meeting they discussed what went wrong this week and what is need to be changed. Nurses also share what each client achieves during the week.
