2014 Age and Gender. Patient’s identified as being discharge with diagnosis of STEMI and NSTEMI during 4th quarter 2014 (October 1, 2014 – December 31, 2014) which met inclusion criteria were N=82. 52 (63.41%) of the 82 were diagnosed as having NSTEMI and 30 (36.59%) of the 82 were diagnosed as having STEMI , “(see Table ___)”.
2015 Classification of MI. 72 of the 104 were diagnosed as having NSTEMI (69.23%) and 32 of the 104 (30.77%) were diagnosed as having STEMI, “(see Table ___)”.
2014 Discharged on Core Measure Medication. Discharge medication reconciliation forms and progress notes were reviewed to determine whether patients were discharged on core measure medication or documentation of reason for exception. Of the 82 patients discharged 4th quarter 2014, 81 (98.78%) were discharged on core measure medication or had sufficient documentation as to why exceptions were made, “(see Table)”.

2014 Medication Compliance at First Office Visit. Hospital discharge medications were compared to office medications at time of first office visit, patients were either compliant or non-compliant. N=71 (86.58%) of the 82 discharged patients were seen in office with 69 (97.18%) compliant with discharge medications at the time of first office visit and 2 (2.82%) were non-compliant, “(see Table ___)”.

Of the 10 patients readmitted within 30 days of discharge, 6 (60%) of those were discharged
