and Mrs. Rasak at their home along with the physical therapist from The Regency. Mr. Rasak was able to get in and out of the car with stand by assistance. He is able to walk using a 50% weight bearing to the right foot and a walker. He was able to maneuver the two small steps to the front door. Once inside the home he was able to sit in his lazy boy chair. The bathrooms in the home and bedroom were evaluated. The following is a list of DME Mr. Rasak will need:
2 raised toilet seats
Front wheeled walker
Gait belt
Grab bars installed in walk in shower
Shower chair
He will need home physical and occupational therapy, 4 hours per day of attendant care. There may be a need for transportation.
I have contacted Incingo for DME; all will be shipped at the latest by Monday. Installation of the grab bars may occur after that. Attendant care will be provided by Acclaimed home care. I have contacted MSW at The Regency. We are targeting a release home on 3/14/17.
Mr. Rasak was discharged home on 3/14/17. Acclaimed homecare has begun to provide attendant care services 4 hours per day. I have spoken with Mrs. Rasak several times. She is very overwhelmed by Mr. Rasak. He wants her to provide all care. She continues to work outside of the home. I have encouraged her to use the attendant care services to make things
