Scenario: A 16 year old football player was running and cut hard to the left and he felt pain in his ankle and came off the field with pain and his cleats were removed which then revealed he had swelling of the injured ankle he was taped up and sent back in.
Subjective:The patient came off the field complaining of a hurt ankle his symptoms were swelling and painful sensation in his ankle.He is male , 16, and has no prior history of injuries.
Objective: The most vital sign to me was swelling around Ankle.It was also noted that the patient felt a sensation on his injured foot.
Assessment: From what was assessed the patient had his first known ankle injury, it was clear to all staff of a swollen ankle because of swelling and all notable symptoms of the injury.
Plan: The plan for the athlete is to ice for 10 minutes then tape his ankle and he will be good to go. His ankle isn't super swollen and has gone down since time of ice. Future education tips to athlete to tape before the game. No referrals just ice the ankle after the
