Susan is a 48-year-old women with a primary disability of severe depression, secondary disabilities include: anxiety, Attention Deficit Hyperactivity Disorder, and possible learning disabilities. This evaluation was requested by her Massachusetts Rehabilitation Counselor Holly Wentzell to determine if Susan would benefit from assistive technology, which would assist her with meeting her vocational goal of receiving her certificate in Human Services.
Functional Capacities:
Susan is able to complete all basic activities of daily living with independence. She is able to ambulate across all surfaces without the utilization of an assistive device. Susan displays significant weakness in the area of reading, reading comprehension, written expression, organization, recall, and time management.

Using the Google calendar will allow Susan to organize and manage her time efficiently. She will be able to use her iPod to synchronize the same calendar to iTunes Tiny Calendar. To support Susan with anxiety, the ATS would like Susan to become familiar with the I Can Be Free application. With this application, Susan can listen to sessions on her iPod, which could help her relax, overcome fears and/or phobias. The application can help modify thoughts and emotions, and will allow her to make immediate modifications to how she perceives and deals with situations. The ATS would like Susan to try the application first to see if this application is a good fit.
