What is comventionally day 1 of mensturation?
The first day of bleeding
Describe the proliferative stage of the menstural cycle
- Early part
- Under the influence of oestradiol from the developing preovulatory follicle - The endometrium grows from 0.5 mm to 3-5 mm - The endometrial glands are narrow and tubular while the stroma becomes progressively less dense |
What triggers the secretory phase of the endometrium?
Following ovulation at day 14, the ovary secretes large amounts of progesterone as well as the oestradiol. The endometrium then ceases proliferation and enters the secretory phase
Describe the secretory stage of the menstural cycle
- The endometrial glands become progressively more tortuous and dilated as secretion of the glycogen rich mucus begins
- The supporting stromal cells become larger and increasingly oedematous |
What happens to the endometrium if pregnancy does not occur?
The decline in progesterone initiates a:
- Shrinking endometrium - Spiral arterioles supplying the endometrium spasm causing ischaemia and stasis - Influx of inflammatory WBC initiating the release of prostaglandins - Tissue collapses leading to bleeding and shedding of the spongiosum layer of the endometrium |
How much blood is lots during mensturation?
Average: 30mL
Range: 10 to 80mL |
Why doesn't normal menstural blood clot?
Because of the presence of fibrinolytic substances
What 3 factors limit menstural blood loss?
- Partial occlusion of the spiral arteries with platelet and fibrin plugs
- Predominance of F series prostaglandins (vasoconstrictors) over E series (vasodilators) - Ovarian follicle growth supplying oestrogen to regenerate the endometrium |
