What is different about women 35-50 y.o.?
fibrocystic changes are at the top of the list, carcinoma is second, papilloma is a possibility (last one)
What is different about women more than 50 y.o.?
carcinoma is first on the list, fibrocystic changes are second, Paget's disease becomes an option (in the middle), and papilloma is still last on the list.
20-40 years old
Cyst formation (with fluid, calcifications, and apocrine metaplasia)

On histo--Huge hole on the left with a smaller one on the right side.
What do the two histo slides look like for FCC?
Many small clumped circles that are gray on the left side.

Many magenta bigger clumps that are on the right side.
A proliferative lesion--double the amount of acini
Fibrosis and glandular crowding
Mimicks cancer, but is benign
Prominent myoepithelial cells
Sclerosing adenosis
Mimicks cancer, both radiologically and grossly.

What does it look like on histo?
florid ductal hyperplasia

Histo--An angled oval on the left side, that is a purple color.
What is important about Atypical Ductal Hyperplasia?
The nuclei are becoming atypical and there is an increased risk of cancer now.
What does a duct carcinoma in situ look like on histo?
A squash on the bottom with a circle to its right. On the left there is a small star.
What does a benign duct cyst look like on histo?
Like bubbles. They are small on the left and progressively get larger.
During the reproductive years
Stromal and glandular hyperplasia
Firm, rubbery, mobile
Benign
Changes with menstrual cycle, and gets smaller and disappears with menopause
Fibroadenoma

On histo--slits and loose fibrous stoma
No proliferation of atypia
Proliferates and is larger than fibroadenomas
Can be malignant--more cellular with increased mitosis
Hematogenous metasasis
phyllodes tumor

On histo--very cellular on the left and ductal n the right.
Solitary lesion usually at the ducts near the nipple
Sometimes with a bloody discharge

What are features of BRCA 1 mutation?
poorly differentiated, medullary, ER and PR negative, and HER2/Neu negative, younger ages, and a higher ovarian cancer risk.
On histo, what does LCIS look like?
a bunch of grapes in a well-circumscribed sphere
What does invasive ductal carcinoma look like of histo?
Light blue stoma with clusters of brown circular cells.
What does invasive lobular carcinoma look like on histo?
cancer cells in a single file or a bunch of thin circular layers.
Large
Rapid growth, but takes long to metastasize.
Less desmoplasia
Frequent mitosis
Most are triple negative
Good prognosis--70% 5year survival
Medullary carcinoma
What does colloid (mucinous) carcinoma look like on histo?
Clear mucin in between S-shaped purple cells.
Intra-epithelial carcinoma of the skin
Eczema involving the nipple
Paget's disease with "halo cells"
Small, well formed tubules
ER positive, HER2/Neu negative
Rarely metastasizes
40 y.o.
"A good cancer to have"
