What is the action/effect of Lanoxin?
Inhibits the sodium-potassium ATPase pump which makes more calcium available for contractile proteins, resulting in increased cardiac output
positive inotropic effect = increases contractile strength of heart
negative chronotropic effect = decreases beats per minute
decreases AV conduction speed
What is a priority assessment before giving Lanoxin? Why?
check apical pulse for one full minute
>60, give med. <60 wait one hour and recheck; still low, call Dr.
Note rate, rhythm, character, monitor ECG continuously during parenteral loading dose
What are the uses of Lanoxin (digoxin)?
Heart failure, atrial fibrillation, atrial flutter, atrial tachycardia, cardiogenic shock, paroxysmal atrial tachycardia
What are the side effects of Lanoxin (digoxin)?
CNS: Headache
CV: Dysrhythmias, hypotension, AV block
EENT: YELLOW-GREEN HALOs
What priority lab value should be checked before giving Lanoxin (digoxin)?
Digoxin level: 0.5-2.0
What should be monitored closely with Lanoxin (digoxin)?
Look for signs of dehydration and decreased urine output (more concentrated)
lower fluid = higher heart rate
What is the therapeutic effect of this drug
decreased wt., edema, pulse, resp. crackles; increased urine output
What is the functional class of Lanoxin (digoxin)
Cardiac glycoside, inotropic, antidysrhythmic
