What are the symptoms of seratonin syndrome and what is it causes by?
caused by overdose of seratonin assoc. drugs such as triptans. Get confusn, irritability, coma, hyperreflexia, rigidity, myoclonus, tremor, hyperthermia and fever, tachycardia
What is the mechanism of the Trigeminal Vascular Hypothesis of headaches?
Stimuli (such as light, hunger, etc) activate the descending trigeminal nucleus which activates the trigeminal nerve. CNV activation causes cerebral vasodilation leading to release of Ser, CGRP and other inflammatory mediators that cause swelling
What is the mechanism of action, theraputic uses, adverse side effects, and contraindications of Dihydroergotamine Mesylate?
5HT1B/1D, 5HT2 receptor agonist that blocks plasma extravasation by the Trigem system. Use w/ anti-emetic. Can cause rebound headaches if overused, ergotism (gangrene, burning of extremities), NV, cramping, vasoconstrxn. Contra: pregos, CVD, CerebroVD, impaired liver/kidney. Do NOT use w/ triptans
What are the uses, side effects, and contraindications of triptans? What are the triptans?
Sumtriptan, Frovatriptan, Naratriptan. 5HT1D agonist that causes cerebral vasocontrxn decreasing release of inflam mediators and blocking trigem nucleus activation. Side affects include jaw/neck tightness, BP elevation, seratonin syndrome. Contra: w/ ergotamines, hemiplegic or basilar migraines, heart problems
What causes seratonin syndrome and what are the symptoms?
Overuse of seratonin associated drugs (triptans, SSRIs, MOA inhibs). Cognative symptoms: confusion, irritability coma. NM symptoms: hyperreflexia, rigidity, myoclonus, temor. Autonomic: fever, profuse sweating, tachy
If a patient has asthma and migraines, what migraine prevention drug should you avoid?
propranolol, nadolol, timolol. Also don't use w/ bradycardic patients
If a patient is obese and has asthma, what migraine prevention drug should you avoid?
tricyclic antidepressants
