Where are blank receipts forms, FD form 1 kept?
Form #1 are kept in the top right drawer of the company desk.
Article XIX Section 5
How is the receipt of Accountable Property recorded?
F&EMSD receipt forms awaiting the attention of the Company Property Officer for entry on the current FD Form 112 shall be kept in the top right hand drawer of the company desk. The current FD Form 112 shall be kept in the executed form file drawer of the company file cabinet.
Article XIX Section 6
When are FD Form 1 kept in the company desk that has been entered on to form 112 removed - and where are they stored?
When proper entry has been made on the FD Form 112, the company’s copy of the Form 1 or Form 42 shall be marked with “E” and filed on ACCO fastener. This ACCO fastener file shall be kept in the top right hand drawer of the company desk until October 1st of each year, at which time it shall be removed from the company desk and placed in the company storerooms.

When accountable property is lost, stolen, destroyed, or damaged beyond repairs – what form is submitted and when?
Form 97 within 24 hours.
Article XIX section 16
What is indicated on a form 1 for equipment that has been loaned or sent for repairs?
When property is temporarily transferred to any other company or division, the Property Officer shall indicate on the form that the property is “loaned”, “for repairs”, or other explanation. When the property is returned, the new form shall indicate “returned from loan”, “returned repaired”, etc.
Article XIX Section 7
How are supplies accepted for sources other than from the fire department?
Before acceptance of supplies delivered from sources other than the F&EMSD, the on-duty Property Officer shall consult with the Supply Management Officer in the Logistics Section. Invoices covering such type of supplies shall be attached to and forwarded with F&EMSD Form 1 to the Logistics Section. Property Officers shall retain the second (pink) copy of the FD Form 1 for their records. It is important that supplies be checked with invoices, if possible, to ensure correctness of delivery.
Article XIX
When are Unserviceable and Surplus Accountable Property collected and what forms are submitted?
The Logistics Section shall collect serviceable and unserviceable accountable property from fire stations semi-annually during the months of April and October. The Property Officers shall submit a Form 17 stating that surplus accountable property is available to be collected. An unsigned Form 1 listing the property shall be attached to the Form 17. A notation shall be entered after each item indicating that the property is serviceable or unserviceable. The Logistics Section will bring the Form 1 when the property is picked up and will leave a signed copy of the form as the Property Officer’s receipt.

What is done when fire department property is left on the fire ground such as a salvage cover?
Execute a Form 1; If possible obtain a signature of responsible person at the scene.
The Property Officers concerned shall take appropriate action to effect the prompt recovery of the property.
Special Reports to the AFCS are necessary only under the following conditions:
When finding the property damaged upon recovery.
When unable to recover property (lost or stolen).

All officers and members below the rank of Battalion Fire Chief shall carry spanners.
Article XIX Section 22
How often are physical inventories taken and what form is used?
They shall be taken annually at the end fo the fiscal year by the Responsible Property Officers – shall be submitted on Oct. 3rd alphabetical list of items charged by class on Form 112.
Article XIX Section 18
When do property officers dispose of official records?
On Oct. 1 of each year, Property Officers shall remove from their files and other places of storage all records that have been maintained in companies longer than the retention period as per Bulletin 50.
