1. Tell the patient there is a risk of angle closure with dilation.
2. Have them sign a consent form.
3. Dilate with 0.5% tropicamide.
Why would we use 0.5% tropicamide to dilate a patient with a risk of angle closure?
1. 0.5% tropicamide can be reversed with pilocarpine.
2. The most dangerous size for pupil dilation is mid-dilation. 0.5% tropicamide will provide a mid-dilated pupil for testing whether or not the angle will close. (a provacative test)
3. Hold the patient to make sure the angle doesn't closes. Check IOP and do gonio after dilation. If the angle closes, we have proven that the angle is occludable.
