What is a stoma?
A stoma is a sergical opening from the inside of the body to the out.
What are some assessments of the stoma?
* skin breakdown and ulceration.
*Leakage(remove to prevent breakdown)
*Color(red or pink)
*Moist
*bloody discharge
What are some signs of stomal ischemia?
*stoma appers blue, black or purplish.
* stoma is blue, black, or purplish
If the stoma appers black or purplish in color this indicates a serious impaiment of blood flow and requires immediete nursing interventions. What are some of these interventions?
*obtain vs and pulse ox.
*counsult current lab results
* Notify provider( may need surgery)
*prep. patient for surgery and notify the surgical team.
