What vulvar precursor lesion is associated with smoking and HPV infection?
Vulvar Intraepithelial Neoplasia
What is the most common malignant neoplasm of the vulva?
Invasive Squamous Cell Carcinoma, commonly preceded by VIN. Spreads stepwise to local lymph nodes first
What non-neoplastic lesion causes thinning of the vulvar skin?
Lichen Sclerosis. Can clinically mimic in-situ neoplasia.
What is the screening test for Cervical Intraepithelial Neoplasia?
Pap Smear. If abnormalities are detected a biopsy must be performed.
What's one problem with a Pap smear?
It has a fairly high false negative rate.
What is the most fatal component of cervical cancer?
Morbidity/mortality from cervical cancer is most commonly due to local effects: obstructed urteters, rectal fistula, etc. Renal failure and infection are common causes of death.
Invasive carcinoma of the endometrium is typically of what type?
Adenocarcinoma
What is the primary risk factor for endometrial cancer?
Increased estrogen. Obesity increases estrogen by peripheral conversion of androgens to estrogens.
Where does endometrial cancer typically spread?
* Pelvic lymph nodes
* Cervix/Vagina
* Ovaries/peritoneum (uncommon)
What is the treatment for endometrial cancer?
Hysterectomy.
What is endometriosis?
Endometrial glands outside the uterus.
What are the clinical SXS of endometriosis?
* Cyclical pelvic pain
* Bleeding/inflammation leading to masses, fallopian tubal obstruction
* Infertility
A "benign smooth mm neoplasm of the uterus"?
Leiomyoma (uterine fibroid)
