What imaging modalities do you rely on for diagnosis of a patent urachus?

The diagnosis can be confirmed with either cystography or an anterograde study through the umbilicus demonstrating free communication with the bladder lumen; although, simple bladder ultrasonography can demonstrate the fluid filled canal.

How does a patent urachus present clinically?
|
Clinically, a patent urachus presents with profuse, watery discharge from the umbilical area, and this can increase as a bladder contraction takes place with voiding. As one would expect, any other increases in intra-abdominal, and hence intravesical, pressure will increase the drainage, such as through crying or straining. However, sometimes the diagnosis may be more subtle with a presentation of an edematous umbilicus and delayed healing of the cord stump

Infected cysts usually present in adulthood with lower abdominal pain and a painful, palpable mass. Patients can have urinary tract infection and irritative voiding symptoms. Ultrasound is useful in confirming the diagnosis with demonstration of the the localized fluid collection between the abdominal wall and the peritoneum. If still unclear, CT imaging can elucidate the anatomy
