OCP Facts
Decreased risk of gonococcal PID
Increased risk of chlamydial infections
Shorter, less painful, scant menses
Decreased risk for ovarian cancer
OCP post pregnancy?
Progestin only -- avoids problems with milk production
Uterine atony
Common cause of postpartum bleeding, find boggy uterus. Treat with uterine massage.
Endometrial cancer
most common gyn malignancy, typically presents with vaginal bleeding in post menopause women. DIC can be performed to help estab dx.
Endometriosis
Pelvic pain, dysmenorrhea, usually women btw 30 and 40.
Cervical cancer
Post coital bleeding, +- discharge. May be asymptomatic until later. Risk factor is sex.
Tubo-ovarian abscess
Risk factor is PID, symptoms of peri-umbilical pains,
Bacterial Vaginosis
Foul smelling, Clue cells = epithelial cells w/smuged borders. Etiology = Gardenella vaginosis. TREAT = Metronidazole
Trichonella
Motile flagellated organism. Men may get urethritis, women -->strawberry cervix. Metro is treatment.
Gonorrhea
G neg diplococci; TX= ceftriaxone + doxy (b/c chlamydia is commonly present)
Methotrexate Tx
For ectopic pregnancy, if b-hcg < 6k. Give 1 dose, repeat b-hcg on day 4 and 7. If has not fallen 15%, give 2nd dose.
