Why do most animals need the endotracheal tube to have a cuff?
The cuff creates a seal, w/o a seal:
*the patient doesn't receive proper concentration of gas & can wake-up
*the gas leaks and pollutes environment
Who does not need an endotracheal tube with a cuff?
*Neonatal puppies & kittens and rabbits (it will tear their trachea)
*procedures that are too short (3-4 minutes: cat neuters); when it takes longer amount of time to intubate than it does for the procedure
What could happen if an endotracheal tube cuff is over-inflated?
Tears trachea; (can lead to subcutaneous emphysema or pneumothorax)
What could happen if an endotracheal tube cuff is under-inflated?
*gas will leak out and expose us
*$$$$$-costs increase when gas is wasted
*animal will not receive proper concentration of gas and can wake-up
What drug is given to cats to stop laryngeal spasm leading to problems w/intubation?
2% Lidocaine topical (use a 1mL syringe w/o needle or a q-tip) used to desensitize the larynx
What are some complications seen w/incorrect endotracheal tube placement?
*Tube can end-up in the esophagus
*Pneumothorax:
*Subcutaneous Emphysema:
Pulse Oximeter:
*What does it measure?
*How is it used?
*Measures: how much oxygen the blood is carrying (% of blood oxygen saturation) & heart rate
*Linguinal clip shines a light through a mucus membrane, light reflected off of RBCs determines oxygen %
*Linguinal clips problematic
*
What are some causes for inaccurate readings w/SpO2?
-Poor probe placement or patient movement
-Hypothermia
-Hypotension
-Tachycardia
EKG or ECG:
*What does it measure?
*What does it help diagnose?
*How is it used?
*Measures the electrical activity of the heart--Does not mean that the heart is beating
*SOME calculate Respiration Rate
*Helps diagnose arrhythmias
Blood Pressure:
What is it?
The pressure exerted by the blood against a vessel wall
Indirect vs Direct Blood Pressure Methods
Indirect Methods:
~non-invasive
~Doppler Ultrasound
