On February 19, 2013, the Department of Human Services, Office of Program Review, Monitoring and Investigation (OPRMI), Fraud Investigations Division (FID) received an anonymous complaint from a concerned citizen alleging Donald Megginson is receiving SNAP/Medical benefits from DHS and resides at 310 Bogota Drive, Ft Washington MD 20744. The complaint stated Mr. Megginson identification was only for the purpose of employment and do not physically reside at 660 Kenyon Street, NW.

On December 3, 2015 Investigator Davis reviewed DHS Automated Client Eligibility Determination System (ACEDS), DHS Document Imaging Management System (DIMS), and determined Mr. Donald Megginson is a single 44 year old male, who received SNAP/Medicaid benefits 12/2012 through 5/2015 for a household composition of 1. His physical address is listed 660 Kenyon St, NW #7, Washington, DC 20019.

located 6230 3rd Street, NW Suite 2, Washington DC 20011, the listed management company for Kenyon House, to verify whether Mr. Megginson was residing 660 Kenyon St while receiving SNAP benefits from DHS. Investigator Davis met with Ms. Heather Coruch, the development manager for Kenyon House, who provided the residential manager's information for further assistance.

Investigator Davis met with Thomas Walsh, the residential manager located at 3628 12th St, NE. Mr. Walsh stated the building was brought and converted into condos November 2014, therefore, any information or documents related to prior tenants he would not be available. However, Mr. Walsh verified that Carlos and Alba DeCruz are the current occupants at 660 Kenyon St, NW #7 and has been since December 19, 2014.

On December 15, 2015 Investigator Davis conducted a home visit to 310 Bogota Drive in attempt to make contact with Mr. Megginson, however, there was no success. Investigator Davis' business card was left at the
