Care also needs to be taken that when making reports that using the client 's name but instead using their initials. This is done so in order to protect the client if the documents should happen to fall in the wrong hands. The same level of care and knowledge needs to be exercised if someone calls in regards to the client’s case that the client has not given permission for discussion to be held with said individual. Information that is the personal opinion of the social worker or pertinent personal information about the client should not be added to the case notes either in case these have to be read in open court. This protects the client and the social worker by information that is not part of the care plan to be a possible breach.…

This can pertain to other agency 's she will work with to achieve her case plan as well. I am applying this theory because all agencies should work around a Trauma Informed plan for clients. Many of the client 's that end up in our care are people who face oppression in their communities or past traumas that occurred prior to them being involved with us. We can 't effectively help a client if we don 't understand where they are coming from and what they have been through. Agency’s should create intervention’s that are more proactive versus…

First, starting with facts, we are aware that Susanna’s therapist told her parents about her diagnoses of borderline personality disorder without her consent. My response to this is the therapist should not have disclosed confidential information and should have provided consultation with Susanna first. This lacked complete disrespect for the privacy of the client. Looking at the code of ethics under 1.07 we know that a Social Worker should not disclose confidential information this includes everyone, including clients parents because she is no longer a minor due to her age. Looking at Policies, I am unaware of what the policy for the therapist might have been during the time period, but we know this would not be acceptable by today’s social work standards.…
