Tonia Toby, CM met with Judith Sevilla (Mother) and John Manalo (Youth) this afternoon at the family home in Union. All FACT initial consents and authorization forms were signed by Mrs. Sevilla and Family Crisis Plan created. CM provided an overview of FACT's model and encouraged the importance of building a child and family team (CFT).
John is a 14 year old Asian male who resides with his Judith Sevilla, (Mother) age 51; Laurence Manalo, (Brother) age 17; and Beatrice Manalo, (Sister) age 9 in Union NJ. John was referred to Rutgers UBHC on 3/20/18 by youth pediatrician after scoring in the severe range on the Beck Depression Inventory. Youth completed intensive evaluations at UBHC. John was diagnosis with F33.3-Major Depressive Disorder, recurrent, severe with psychotic symptoms. On 4/30 18 John saw a Dr. Manfred K Obi, MD who prescribed Sertraline 25 mg 1 tab daily and Prednisone 20 mg 2 tabs daily. According to Mrs. Sevilla, Mrs. Alyssa Cariani, LPC from Rutgers UBHC in Newark referred John to FACT because she felt that John needs more intensive services. …show more content…
John attempted suicide on at least two occasions. Mrs. Sevilla report being in denial about youth mental health challenges. She reports that it wasn’t until youth started experiencing psychosis in the form of auditory hallucinations and depersonalization. Youth experience physical and verbal abuse from his father which led to DCP&P involvement with the family back in April 2018. John also witness domestic violence between his parents in the past and he also reported being physically abused by his older siblings. Youth parents are currently divorce but custody has not been established. Mrs. Sevilla wants sole custody of her children but is afraid due to her ex-husband abusive behaviors. Youth father is not included in his treatment due to Mrs. Sevilla fear that he will not allow youth to receive
