Pt. was assigned to this writer's caseload on 8/22/16 and he has shown good participating during individual sessions. Patient has not earned take home privileges during the last quarter because his random UDS/urine sample submitted was "diluted". Pt. remains in compliance with monthly required counseling. Pt. is confident that he can maintain his clean and sober life after having relapsed briefly 9 moths ago. Pt. was encouraged to stay away from old haunts and old friends from his using days. Pt. denied any current side effects from his methadone medication and he is taking it as prescribed. During the next quarter, Counselor will teach Pt. the important of recognizing the things that might trigger him to use again before it happens.
