The patient is a 19-year-old gentleman who tells me he has been having a sore throat and congestion for the past two weeks. He was also having a cough that would wake him at night. He states the cough now is better but he continues with copious rhinorrhea and postnasal drip. He also has a tickle in his throat, but it does not feel severe. He denies any fevers.
The patient wants me to look at a lesion on his left toe #5. He states it has been there for quite some time. At this point, it is getting painful to walk on.
The patient also tells me he had a puncture wound on his left hand, which occurred from a very rusty grate at work. He does wonder …show more content…
We will have patient use Allegra-D, Flonase two sprays each naris daily, saline nasal spray. He will drink copious fluids and fluids and try to get rest. He will let me know if symptoms do not start to improve. He is having no sinus pain to think that it is a sinus infection, at this point.
Wart, left toe #5. This was shaved with a sterile blade. He did have punctate bleeding with this. There is no pigmentation to this lesion. Liquid nitrogen was used x2 freezes. I did discuss risks including possibility of blistering and pain. I discussed care of what to do if this is the case. I also discussed that if there is any lesion left after about three weeks, he should come in for re-treatment.
Puncture wound, left hand, very rusty, metal grate. I do think we should give him a tetanus shot since it has been greater than five years since he has had one. TD was administered today.
