Missing Doctor Referral

I’m writing to you today because my insurance does not cover the $119.00 charge from the Greater Dallas Radiology Assoc because of the missing doctor referral. In Jan 8,2016 I had a checkup with the doctor. However, the doctor told me that I need to do a mammogram. Four weeks later, a nurse from the doctor call me and given me the name of the of the doctor and the hospital for setting up the appointment. At the time, I did not know that I need a doctor referral so I did not ask for it, and the nurse did not give me the doctor referral. Please review the claim again so that I don’t have to pay the $119.00.
