These people can still process thoughts normally as long as brain function has not been affected from the incident. The issue is that, although have the ability to think, they cannot move parts of their body which sometimes includes the mouth, making them unable to speak or share what they are thinking. A great example of this can be found by looking at the life of Stephen Hawking, the famous theoretical physicist who suffers with a form of motor-neuron disease called amyotrophic lateral sclerosis. Hawking has an incredible mind and even when he ends up in a wheelchair, unable to move from his disease, his brain still functions at a normal rate. Although he can no longer move, he has a device that speaks electronically for him, allowing for people to see that he still has the same genius
