Resident John Shock Case Study

Reporting party (RP) Residents were not being awakened for meals if the residents are asleep during meals the caregivers don't wake them. There is no food provided for the residents once they wake. Hospice residents who are sleep and don't make it into the dining hall they are not fed. Resident John Shock would talk about bodily functions making the other residents uncomfortable. He is removed from the dining area sent to his room and not served food. The RPs stated assistant manager has been observed and heard yelling at residents. On one occasion Wanda was observed yelling across the room at John Shock when requested an additional desert "you know your family has you on a diet" in a loud angry tone. Regularly there are only 2-3 sandwiches available to residents who wake up during the night for a snack. The manager is not concern regarding a resident's safety. A resident named Betty Whitaker is a fall risk and should not walk, however the resident was observed walking with her wheelchair. Wanda observed the resident walking but did not stop talking to assist the resident she informed a caregiver to "get Betty." Residents have access to sharp knives in the dining area. The medication staff have been observed administering medications with pudding without a doctors order. Wanda overheard making inappropriate statement to a new hire stating she didn't want to pick up another residents from the dirt in reference to a recent suicide of a resident who threw himself from the second floor. The comment was made in the presence of residents. The RP disclosed caregivers do not complete their rounds to check on resident resulting in residents sitting in there incontinence briefs for 6-8 hours.
