8/19/2015: The social worker was asked by the nurse to assist a patient, per Dr. R. Sundaram. The patient was requesting a letter to state his competence in supervision of his two children. The Social Worker is not qualified to address a letter such as this and consulted her supervisor about this matter. The Clinical Services Officer, Nursing Service Director, LPN – Team Lead, Dr. R. Sundaram and the social worker met to discuss this patient’s case. It was substantiate that there is not enough evidence available to compose any type of letter addressing the issues that the patient required, because the patient has only been seen twice at our clinic and only first established care on 07/31/2015.…

When I think of the term ethical social work practice, it encompasses all the responsibilities’ of a social worker. Although many values from the National Association of Social Workers (NASW) Code of Ethics are applicable, the values that are most represented by this policy are the importance of human relationships, dignity and worth of the person, and social justice. Unfortunately, I have encountered a situation where there was suspicion of sexual abuse of a child. An employee of mine reported a situation they felt was inappropriate. The employee was working with a young man who received our services who had an accident while in the community. The employee returned the young man to the home to change clothing and found the grandfather of the child laying in bed with the younger 5 year old sister. The employee reported he could not verify any wrong-doing was occurring, however, neither the grandfather nor the girl stepped out from underneath the covers.…
The primary duty of a health care provider is to build a trustworthy relationship with their patients. This includes protecting the patient’s health information and concerns. In this case, confidentiality is breached due to both therapists, Dr. Knowles and Dr. Thomas, because of discussing their patients’ information without their consent. I believe that it is immoral for Therman to not take any precautions when he was engaging with Margo, since HIV is a big deal in the society. Self Determination Right to self-determination is clearly stated in the Code of Ethics as a social worker’s ethical responsibility to their client. Self-determination means social workers will respect and promote the right of the client’s self-determination as well as assist them in an effort to identify and clarify their goals (NASW, 2008). In this case, the only time a social worker is able to limit the clients self-determination is when the client may pose a danger to self or others. The benefits of this decision would be that I would clearly be acknowledging my friend’s right to privacy, and to choose how to run her own life. However, at the end of the day, I would be left with the knowledge that many of the people with whom I associate in the community could be potentially in danger of not only being infected with AIDS, but also at risk of spreading the disease. Another fact to take into consideration if I were to choose this course of action, is what is required of the doctor during and after this appointment.
