This data set, which contains 43 variables across all participants, is available to researchers upon request. The primary investigator asked the United States Office of Special Education and Rehabilitative Services for this data and signed the required agreements in June of 2014. Grand Street Settlement (GSS) is an organization which provides sections of New York City with various social services, including Early Head Start (EHS). EHS is a federally-funded program meant to close the gap in childhood outcomes between those in poverty and those in the rest of the population. It consists of programs which connect families to a network of needed educational, medical, and other social services and which provide a place that fosters their ongoing development. The National Association of Special Education Teachers (NASET) has explained the steps necessary to identify a high-risk student. Additionally, they have offered referral techniques and created logical frameworks for multidisciplinary teams to follow. Similar in context to the first case above, the implications of Timothy v. Rochester are groundbreaking. Current students with disabilities are the recipients of Individualized Education Programs (IEP). Post-Secondary Goals and Self-Advocacy This is an instructional project that includes measurable postsecondary goals that are required for all students that have verified disabilities. It was mandated through the IDEA reenactment of 2004, that transition planning begins by the age of 16. The transition services for students with learning disabilities will have a focus on helping the students gain a standard diploma, self-advocacy, post-secondary goals, and detailed plan so that when transitioning into adulthood and life after high school they will be more adapt to be accepted into a program or workplace that will meet each student’s needs. The Meadows Foundation has taken some part in many projects in Texas that have built playgrounds for children with disabilities. One of the Meadows Foundation main initiatives serves towards mental health. Society has made substantial progress in terms of the acceptance and social treatment of individuals with intellectual and developmental disabilities (IDD). This can be most clearly observed by the deinstitutionalization movement in the 1970s, where individuals with IDD were no longer sent away to institutions because they were considered unsuitable for society. Rather, they began living, working, and interacting within their own communities. The TA Requestor, Ms. Shauna King-Simms, Director of Transitional Education Programs at the Kentucky Community and Technical College System (KCTCS) heard a description of Crittenton Women’s Union’s (CWU’s) Mobility Mentoring® Model for case management at the Regions I-IV Roundtable in Philadelphia, Pennsylvania in mid-August 2014.
