    Patient ambulates ad lib without difficulty, gait even and steady. Negative Romberg test. The curvature of spine normal with double-S shape, no deformities or limitations to movement. Lumbar and thoracic vertebral non-tender with palpation. Medication is Hydrocodone with Tylenol 10-325 mg, one tab PO every 6 hours as needed for musculoskeletal pain. States since swelling went down he has not required any pain medication. No surgical history or laboratory data related to the musculoskeletal system. Light touch sensation to upper arms identified without difficulty. Smooth and accurate repetitive finger to nose movements. Deferred having heel ran down each shin due to lower extremity edema and reported tenderness with light palpitation. No muscle atrophy or tremors in any extremities. Equipment unavailable, tendon reflexes not tested. Trazodone 100 mg PO at bedtime daily for insomnia. No surgical history related to the neurological system. Laboratory data, NA 139, K 3.7, CL 108, CO2 26, anion gap 8.7, GLU 124. History of STI's denied. Currently, voiding large amounts of clear yellow urine at frequent intervals. 1200 ml urine output noted in 4-hour duration. Scrotal edema not present per patient. No medications, surgeries or laboratory data related to the male reproductive system. Plan Diagnosis A. Impaired Skin Integrity; open popliteal wound related to excess fluid volume AEB generalized edema, damaged integumentary tissue, and discoloration/erythema of lower extremities.…
