Frank Bivins completed a Medical Source Statement of Ability To Do Work-Related Activities (Physical and Mental). He stated the claimant could occasionally lift up to 10 pounds but could not carry anything. She can sit 10 minutes, stand 15 minutes and walk 10 minutes at one time without interruption. She can sit 15 minutes, stand 20 minutes and walk 10 minutes total in an eight-hour workday. The rest of the time, she would need to be laying down. Dr. Bivins further stated that the claimant could never reach overhead or push and pull. She could occasionally reach in all other direction, handle, finger, and feel because of the pain in her fingers. She could occasionally operate foot controls. She can never climb stairs, ramps, ladders, or scaffolds. She can never balance, stoop, kneel, crouch, or crawl. She can never be around unprotected heights, moving mechanical parts, humidity and wetness, dust, odors, fumes, and pulmonary irritants, extreme cold, extreme heat, or vibrations. She can frequently operate a motor vehicle. Dr. Bivins also opined that she had an extreme limitation in understanding and remembering simple instructions, carrying out simple instructions, the ability to make judgments on simple work-related decisions, understand and remember complex instructions, carry out complex instructions, the ability to make judgments on complex work-related decisions. Bivins completed a treating source statement on April 10, 2018. He stated the claimant could stand/walk a total of one hour in an eight-hour period and sit a total of one hour in an eight-hour period. She would need to alternate sitting and standing every 10 minutes. She had the ability to lift less than five pounds on an occasional basis and on a frequent basis. She cannot walk at a normal pace and cannot bend, stoop, squat, kneel, reach overhead, climb stairs, fine and gross manipulation of both hands at all during an eight-hour work shift. In addition, Dr. Bivins opined the claimant would be off task 90% of the time due to having to take unscheduled work breaks or the pain would make her unreliable in performance of even simple work tasks. She would be unable to work or absent due to her medical conditions four or more days a month
