Because of inadequate quality concerning follow-up care services, this may result in poor clinical outcomes. Care takers of individuals diagnosed with dementia play an important role in achieving successful transitional care. Thus they depend on clearly constructed and documented discharge plans. However, as a result of inadequate service planning and continuity between community and residential care settings, family support can be significantly impaired. This can cause an enormous amount of stress and exhaustion toward care takers as they are expected to take responsibility for their (the individual diagnosed with dementia) physical, emotional, social care, and medical follow-ups.
Non-Music Therapy Treatment Trends The use of pharmacological drugs appears to be a common non-music therapy trend. Seitz et. al (2012), conducted a systematic review of the common types of drugs prescribed by doctors to individuals diagnosed with dementia. According to the results, the most common medications evaluated in studies were atypical antipsychotics, typical antipsychotics, anticonvulsants, and cholinesterase inhibitors (Seitz et. al, 2012).
