Based on the submitted documentation, the main medical issues, in this case, are acute cystitis with hematuria, left-hand numbness, left-sided neck pain, cervical disc disease, dysmenorrhea, premenstrual dysphoric disorder (PMDD), and bacterial vaginosis (BV). The claimant's workup includes an MRI of the cervical spine with and without contrast which revealed a reversal of the normal cervical lordosis centered at C5-C6, with minimal endplate degenerative changes and disc bulge. There was no significant central canal stenosis or neural foraminal stenosis. The claimant had been admitted from 03/14/17 through 03/15/17 due to symptoms consistent with stroke with left-sided weakness and numbness. She had also been found to have cervical disc disease. Norco was prescribed. Physical therapy was suggested. She had recovered sufficiently and was able to return to work on 04/07/17.
