Also there is leaden paralysis identified by feelings of heavy arms and legs. She also exhibits impairments when facing rejection. The symptoms are severe as she presents most of the symptoms of each category as well as the length of time she has experience the symptoms. The symptoms also have a hinder her ability to work, complete school and affects her relationships.
I would also give Ellen a secondary diagnosis of F41.0 Panic Disorder or a Panic Attack Specifier. However, more information is needed in regards to the panic attacks in order to be able to complete a diagnosis. Per the textbook, it is common for BD clients to have dual diagnosis associated with ADHD and anxiety disorders with a 10.6% to 62.5% panic disorder rate.
When making this diagnosis, the major consideration was whether Ellen had experience episodes of mania or hypomania. What I took into consideration was the fact that during these episodes of elevated moods, Ellen’s daily functioning was not severely impacted or included any psychotic symptoms that warranted a hospitalization or engaged is severely risky behaviors that could potentially get her in trouble.
