In nursing, critical thinking for clinical decision-making is the ability to think in a systematic and logical manner with openness to question and reflect on the reasoning process used to ensure safe nursing practice and quality care (Wilkinson,2011, p.25). There are many residents living in there, the area that I work at only one register nurse taking in charge. We always having a busy day. During the few days monitor, I found out as a nurse you need to have a well time management and planning. In the nursing home, we usually document things in the computer system. Before the shift started, we need to check the work -log to find out what should we finished today, this requires well time organizing and planning. In my shift, we have medical around in 8am, 12am, and 2pm, and wound assessments, DRs review and prepared handover. As a nurse, you need to know how to assign work task allocated and finished them on time. An informal nursing care plan is a strategy for action that exists in the mind (Ulrich and Canale, 2005, p.40).And formal nursing plan is a legal writing document or computerized guide that organizes information about the clients care(Ulrich and Canale, 2005,p.42).Initial planning is developed by the nurse who performs the admission history and physical assessment (Rezler and Tichy, 1985,p.12).
