There are many barriers affect to successful exclusive breastfeeding such as preterm infants, breastfeeding difficulty due to ineffective sucking that can decrease milk production and lead to the limit of milk supply to the preterm infant and could intervene the successful breastfeeding. Moreover, prolong hospitalization results in more mother–infant separation that reduce the opportunity of breastfeeding. (Nagulesapillai et al., 2013). The other factors include painful nipples, cracked nipples, breast engorgements, leaking milk, mastitis and breast abscess are also make breastfeeding problems (Cooke, Sheehan, & Schmied, 2003; Labarere et al., 2012). All of above are barriers which come from both mother and infant and can occur at any time during breastfeeding. Moreover promotes a relationship with between infants and mother and promotes family members involvement in the program will lead to the success in 6 months exclusive breastfeeding. The researcher believes that this program will help mothers of preterm infants to receive the appropriate knowledge and information in breastfeeding and have greater confidence and motivation which leads to the development in capabilities and changing health behavior to successful exclusive breastfeeding. Furthermore, it also helps increase the rate of exclusive breastfeeding resulting to good benefits to preterm infant. The conceptual framework is shown in Figure 2.
