A unit of leukocyte depleted Red Blood Cells (PRBC) contains:

Volume 259ml +/- 15 ml (>220ml)


Haemoglobin 48 +\- 5 (g\unit)

Maximum shelf life of PRBC

42 days

Leukodepletion reduces risk of:

Cytomegalovirus (CMV).


And other transfusion reactions.

The two blood grouping systems:

ABO


Rhesus (Rh)

The universal donor blood group:

O neg


If supply shortages then O pos can be administered except in: females of childbearing age with unknown blood type. And children that are known to be Rh neg.

PRBC must be stored at temp:

2-6 degrees Celsius

Unstable temp may result in:

Damaged RBC


Promotion of bacterial growth

Maximum time out of blood shipper/carrier:

30 minutes (discard after this)

Indications for PRBC

Traumatic cardiac arrest


Severe TBI with associated haemorrhagic hypovolaemia and SBP <120mmHg


Haemorrhagic hypovolaemia Non TBI SBP < 70 altered conscious state.

Contraindication to PRBC

Religious objections to blood products


Children <18 years (not married) without parental consent.

Administration via

IO or IV via blood filter giving set

For rapid administration:

Get crew to squeeze bag


Or: pressure bag.

Prime line and flush after blood with:

Normal saline

Monitor these signs every 15/60

Temp

Following 4 units of PRBC:

Administer Calcium Gluconate 4.4mmol (2gm) IV over 2-5 minutes.

Don’t administer this fluid concurrently:

Dextrose 5%

Transfusion reactions occur in:

1-3% of patients
