– Superscription: – Name , address, age, date (e.g. printed already) – Inscription (body): e.g. Penicillin VK Tablets 500 mg – Name – dose form – strength of medication – Subscription: quantity to be dispensed – E.g. Disp: Thirty (30) tablets – Signatura (transcription) – Instructions to patient: - e.g. Sig: take one tablet by mouth every 6 hours for dental infection – Write if refills are needed and signature – E.g. – Motrin tablets 600mg (concentration is not needed when there is only one form in the market) – Disp: twenty (20) tablets – Sig: Take one tablet by mouth every 6 hours for pain and swelling as needed – Refills: 2 Signature – DEA (Drug Enforcement Agency) number is only required if it is a controlled substance |
