What is Schilling classification?
It was established when Victor Schilling, a german hematologist, noticed that in many diseases there is an increase in the percentage of immature neutrophils.
What exactly was the first chart that he developed?
It reported the percentages of the the different neutrophilic cell types and in part was arranged as follows:
Under normal conditions, myelocytes= 0 metamyelocytes= 0 Band cells= 2-6 Segmented cells= 55-75. |
What can one notice about this chart?
The immature cells are on the left side of the chart. If percentages of immature cells increased, Schilling referred to it as a "shift to the left".
What is a degenerative shift to the left?
When a shift to the left is accompanied by a low white cell count.
A degenerative shift to the left is seen in typhoid fever. TRUE/FALSE
TRUE. It is caused by a depression of the cell factories in the bone marrow.
What is a regenerative shift to the left?
When the shift to the left is accompanied by a high white cell count.
A regenerative shift is seen in pneumonia. TRUE/FALSE
TRUE. There is a stimulus of the marrow cell factories.
What is the significance of a shift to the right?
It implies an increase in hypersegmented neutrophils.
When is a shift to the right seen?
It can be seen in pernicious anemia, caused by the malabsorption of vit. B12.
Normal values for differential counts vary with the age of the patient. TRUE/FALSE
TRUE. Childrens blood normally contains 0-2% basophils, 0-5% eosinophils, 25% to 75% neutrophils, 30-70% lymphocytes, and 0-8% monocytes.
Leukocytosis with an increase in the percentage of neutrophils indicates a severe infection with a good response of the bone marrow. TRUE/FALSE
TRUE.
What should a normal Schilling classification chart look like?
Neutrophilic myelocytes-0
'' '' metamyelocytes-0 neutrophilic band cells-2-6 '' '' segmented cells-55-75 lymphocytes-20-35 monocytes-2-6 eosinophilic segmented cells-1-3 basophilic segmented cells-0-1 |
What is the significance of the appearance of numerous juvenile cells?
Metamyelocytes, indicates irritation of the bone marrow with regeneration.
A falling white cell count with the number and maturity of neutrophils progressing means what?
Indicates recovery.
In tbc, an increase in monocytes indicates activity in the infected area. TRUE/FALSE
TRUE. An increase in lymphocytes indicates healing.
Eosinophils increase in allergic and parasitic infections. TRUE/FALSE
TRUE.
